DISABLED PLACARDS
Short Term
The short term Disabled Placard can only be valid for a maximum of 12 months. Individuals who are suffering from short term disabilities can apply.
A doctor must complete a disabled placard application before you can obtain a short term disabled placard. After your doctor completes the Disabled Placard application, come into the Southland Registration office and submit it to one of our friendly staff. They can assist you in obtaining your short term Disabled Placard.

Long Term
Long term Disabled Placards are valid for a maximum of five years. When your Disabled Placard is expired you can apply to renew it.
To apply for a long term Disabled Placard, your doctor must complete an application form on your behalf. After the completion of the Disabled Placard form, come into the Southland Registration office and submit your application to one of our friendly staff members. They can assist you in obtaining your long term Disabled Placard.
You may ask your doctor to provide you with an application for a permanent disability placard. Permanent placards still need to be renewed every five years; however, they do not require you to see a doctor to do so.

If the disabled applicant is unable to attend, a friend or relative can apply for the disabled placard on their behalf. You will need to provide the application form and an authorization form signed by the applicant.
